Participants who complete the seminar receive a certificate of attendance.

Provide your contact information and select the available seminar date in our secure Jotform.

Submit seminar payment
Use the secure Square payment link when it becomes available. Seminar and NCCDP application fees are separate.

Receive confirmation
Williams Signature Care will confirm your registration and send seminar details to the email you provide.
The seminar and
certification are separate.
Anyone may attend the ADDC Seminar. Completion of the seminar is a required step for eligible professionals pursuing the Certified Dementia Practitioner (CDP) credential.
CDP eligibility, application, approval, and certification fees are handled separately by NCCDP. Seminar attendance does not by itself guarantee certification.
